This report is also referred to as the First Time Enterers (FTE) Action Report. It must be enabled by an administrator through Administration > Properties > School Nurse.
This report displays a breakdown of grade level, vaccine family names, vaccinations (number of doses) needed, recommended date, minimum valid date, per student with SIIS ID, specific grade level, physician name, exempt status, guardian, birthdate, home phone, physician phone, etc., with subtotals per grade and grand totals at the bottom.
To run this report, School Nurse users can click the Reports > School Reports > Immunization Detail Action Report link. The Immunization Detail Action Report page opens. Enter the search criteria for the school and click Search. The search results appear in that section toward the bottom of the page.
The available report parameters are as follows:

To search for a school in a specific school district, select it from the drop-down list. |

Select All (default), Public Only, or Private Only. |
Name |
If the School Nurse user is only associated with one school or if a school was previously selected, it is displayed in this field. |
Series |
Select a series from the drop-down list. This field is required. This is a state-configurable option via Administration > School Nurse Settings > Define Report Series. |
