This report provides per-provider details for New Providers, Total Patients Loaded, and Total Vaccinations Loaded.
This report may take a while to run, so you can click the Schedule button to schedule the report to run during off-peak hours. See Schedule a Report for more information.
To run this report immediately, authorized users can click the Reports > State Reports > Vaccine for Children (VFC) > Provider Activity (Detail) link. The Provider Activity Report (Detail) page opens. Enter the criteria and click Create Report. The report displays in a separate browser tab and can be printed using the browser's print function.
The available report criteria are as follows:

Select the year from the drop-down list. The current year is the default. |
Provider Type |
To limit the report to a specific provider type, select it from the drop-down list. |

Select one of the report subjects:
