Patient Record (Arizona)

This report combines the Patient Record Summary and Forecast reports. Confidential information is optional based on the user's selection. The summary of the report shows Td split out from the rest of the DTaP/DTP/DT family. It also shows PPD and chickenpox history. The forecast table prints below the summary table, listing the next show due. This report is used by school nurses and other medical staff.

Note that this report is similar to the Patient Record report available from the Reports > Patient Record link, but with fewer options to include in the report.

To run a patient record report, authorized users must first select at least an Organization (IRMS) and then search for a patient. After selecting a patient from the Search Results list (by clicking on the name), the Reports > State Reports > Patient Record link becomes available (to authorized users). Click it to open the Print Patient Record page.

Select the desired options and click Create Report to run the report and display it in a separate browser tab. To print the report, use the browser's print function.

The available report options are as follows:

Do Not Include Confidential Information
Include Confidential Information

If the user has permission to view confidential patient information, (s)he can select the option to include the confidential information in the report, or select the option to not include the confidential information. If the report is to be printed, consider the HIPAA implications of including confidential information in a printed report.
