This report populates the WV Vaccines for Children Program - Provider Profile form and is used for vaccine projections. Part A counts all publicly supplied vaccines regardless of patient VFC eligibility. Part B counts all VFC-eligible vaccines only.
To run this report, click the Reports > State Reports > Vaccine for Children (VFC) > Vaccine Need link. The VFC Vaccine Need Report page opens. Enter the criteria and click Create Report. The report displays in a separate browser tab and can be downloaded and/or printed.
The available report criteria are as follows:

Period Begin Date |
Enter the beginning date. This field is required. |
Organization
(IRMS) |
Select either an Organization (IRMS) or Group from the related drop-down list. This field is required. |
Facility |
To limit the report to one specific Facility, select it from the drop-down list. |
VFC PIN |
To limit the report to one specific VFC PIN, select it from the drop-down list (after which the checkbox is automatically selected). |
