Childcare Immunization Report (Arizona)

This report allows Child Care users to view a report that aggregates all of their center's immunization totals for a specified list of vaccines to report this information to the state. Click the Reports > Child Care Reports > Childcare Immunization Report link to create the report. The spreadsheet file downloads based on the Child Care user's Organization (IRMS) and Facility.

Other authorized users can schedule this report by clicking the Reports > State Reports link and then the Schedule button next to the Other > Childcare Immunization Report on the State Reports page. See Schedule a Report for more information.
