This report lists the patient, requestor, and request date for the 402P forms. You must first search for and select a patient.
To run this report, click the Reports > State Reports > Patient Specific > Health Information Request for [Patient Name] link. The Health Information Requests for [Patient Name] report opens in a new browser tab. To print the report, use the browser's print function.
