Login/Logout of PHC-Hub

Use the credentials provided by your administrator to log in to PHC-Hub. If your state uses the SSO-enabled PHC-Hub application, follow the SSO login instructions. SSO (single sign-on) allows you to log in to multiple applications from one login screen. Otherwise, if your PHC-Hub application is not SSO-enabled, follow the non-SSO login instructions. (You can tell which version of PHC-Hub you use by looking at the login screen.)

SSO Log In

If your state uses the SSO-enabled PHC-Hub application, your login page looks similar to this:

1. Open the link to the PHC-Hub application in your browser
2. Enter your user ID and password in the User Id and Password fields.
3. Click Login.
4. When PHC-Hub opens, select the Provider database.

Non-SSO Log In

If your PHC-Hub application login page looks similar to the image below, follow these steps.

1. From the menu, click Main > Login.
2. In the Provider field, select your Provider database.
3. In the User Id and Password fields, enter your user ID and password.
4. Click Login.

Log Out

Log out from the Main menu (Main > Logout).

Switch Providers

1. From the menu, click Main > Switch Provider.
2. Select the Provider from the drop-down list.
3. Click Submit.

Return to the STC Application Page (SSO)

If your state uses the SSO-enabled PHC-Hub application, the PHC-Hub menu includes a Main > Application Dashboard link. Click the link to return to the main SSO dashboard page, where you can access other available STC products such as IWeb, SMaRT AFIX, STC | iQ, and VOMS.
