CDC Export Mappings

The CDC Export Mappings report displays the CDC reporting ID, Sentinel code, condition description, and mode (such as NETSS or Varicella), and allows authorized users to edit or delete the mapping values.

To run this report, select the ELR - CDC Reporting > CDC Export Mappings menu link. The CDC Export Mapping page opens, displaying the list of current mappings. The list can be sorted by clicking on the column header. Under the list of current mappings, the number of mappings displayed on the page is listed (for example, Showing 1 to 10 of 123 entries) and on the right side of that line are previous and forward arrows; click the arrows as needed to display additional mappings.

To create a new CDC export mapping, click the Create Mapping button. See the Create a CDC Export Mapping section below for more information.

To export the list of current mappings, click the CSV link in the Export options line at the bottom of the list. This list of current mappings is exported as a CSV file.

Create a CDC Export Mapping

To add a new CDC export mapping to the application, click the ELR - CDC Reporting > CDC Export Mappings menu link. On the CDC Export Mapping page that opens, click the Create Mapping button. The Create Mapping page opens. Enter at least the required fields (in red text and marked with an asterisk) and click Create Mapping. Click Cancel to return to the CDC Export Mapping page without saving any changes.

Field	Description
CDC Reporting ID *	Enter the reporting ID assigned by the CDC. This field is required.
Condition *	Select the condition from the drop-down list. This field is required.
Export Mode	Select the export mode from the drop-down list.
Core Reportable	Select this option to report cases of this condition.
Verification Reportable (NETSS Only)	Select this option to send the verification record of this condition.
Case Classification	To include a case classification, select one or more of the options. Examples: Report Confirmed Cases - This indicator determines that the patient has this disease. When the investigation is complete, the Investigation Status is set Completed or Closed Report Probable Cases - This is an initial indicator that the patient is likely to have this disease and further investigation is warranted Report Suspect Cases - This is an initial indicator that the patient may have this disease and further investigation is warranted Report Unknown Cases - This indicates, the person making the referral could not determine the likeliness. Follow-up is warranted
Investigation Status	To only send when the investigation status is completed or closed, select this option.

After clicking the Create Mapping button, the CDC Export Mapping page reopens with the newly added mapping added to the list.

Edit a CDC Export Mapping

To edit a CDC export mapping, first click the ELR - CDC Export Mappings > CDC Export Mappings menu link, then search for the mapping and/or located the mapping in the list of CDC export mappings. Click the Edit button for the mapping in the list. The Edit Mapping page opens with the details pre-populated in the fields. Make any necessary modifications and click Submit Changes when finished. To cancel without saving any changes, click Cancel. To reset the fields to the saved data for the mapping, click Reset.

Field	Description
CDC Reporting ID *	The reporting ID assigned by the CDC.
Sentinel Disease Code	The disease code assigned in the Sentinel.
Condition *	Select the condition from the drop-down list. This field is required.
Export Mode	Select the export mode from the drop-down list.
Core Reportable	Select this option to include the reportable condition in a NETSS Export. The reportable condition can be exported via NETSS Export as a Core Record.
Verification Reportable (NETSS Only)	Select this option to send the verification record of this condition.
Case Classification	To include a case classification, select one or more of the options. Examples: Report Confirmed Cases - This indicator determines that the patient has this disease. Column/Option	Description
Show ___ entries	Select from the drop-down list the number of entries to display on the page: 10, 25, 50 or 100.
Search	Enter a few characters of the sentinel code, condition, or mode in the search field to narrow the list of diseases.
CDC Reporting ID	The reporting ID assigned by the CDC.
Sentinel Code	The code assigned in the Sentinel.
Condition	The name of the condition.
Mode	The export mode.
Edit	Click this button to edit the CDC export mapping. See Edit a CDC Export Mapping.
Delete	Click this button to delete the mapping.
