Search for a Physician Card

Authorized users can conduct a search either from Sentinel or from the Physician Card Entry application.

To conduct a search for a Physician Card in Sentinel, first click on the Home > Investigations tab, then click the Search button. The Investigation Search page opens. Select the Physician Card option in the Investigation Search drop-down list. The physician card search section opens at the bottom of the page. Enter the search criteria and click Search. The main Investigations tab page opens with the search results for all the facilities. Click the X button to return to the main Investigations tab page.

To conduct a search for a Physician Card from the Physician Card Data Entry application, select the Physician Card Search option from the Physician Reporting menu. The Physician Card Search page opens. Enter the search criteria and click Search. The main Physician Card tab page opens with the search results for the facility. Click the X button to return to the Physician Card List page.

The following categories of search criteria fields are available on the Physician Card search page:

  • Core: Patient Demographic
  • Core: Case General Information
  • Core: Investigation Address
  • Core: Investigation Dates
  • Core: Outbreak
  • Patient Information
  • Disease or Condition Information
  • Mycobacterial Disease Information
  • If gonorrhea, chlamydia, or syphilis (including congenital), provide the following treatment information
  • Hepatitis Specific Information
  • Lyme: Symptoms And Signs of Current Episode (Please Mark Each Question)
  • Lyme: Other History
  • Lyme: Laboratory Results
  • Lab Test Results
  • Person Providing Report
  • Attending Physician
  • Comments or Additional Information
