Demographics Tab

The Demographics tab page allows you to view or edit the following information:

The available fields, options, and buttons on the Demographics tab page are as follows:

Field/Option/Button Description

Drop-down arrow button
Select one of the following options to perform an action:
  • Case Definition - Opens the disease information in another browser tab or window and displays the Centers for Disease Control and Prevention (CDC) or other source, such as Red Book Online
  • Vaccination Records - Opens the patient vaccination record from the registry application (if it exists) in another browser tab or window. This option is available only for human cases. See View Vaccination Records
  • Print Investigation - Select this option to print a summary of the case information. See Print a Case Investigation
  • Print Case Report - Select this option to print the case reportable disease and/or condition report. This option is available only for human cases
  • Print Physician Card - Select this option to print the case details entered by the physician card user. This option is available when the case has been entered by a physician card user
  • Debug HL7 CDC Export - Opens a new browser tab or window to display the fields that are being sent to the CDC. This option is available when the disease of the opened investigation is exported using any other method than NETSS export (configured in CDC Export Mapping) and is displayed when the HL7 CDC Debug Export permission is granted in the Administrative Debug Permissions section.   • Debug Database for Patient - This option is visible when the Debug Mode permission is granted in the Administrative Debug Permissions section. It displays the database information about the patient. Contact STC to use this
  • Debug Database for Case - This option is visible when the Debug Mode permission is granted in the Administrative Debug Permissions section. It displays the database information about the case. Contact STC to use this

Demographics Section:

To display this section on the Demographics tab page, click the header.

Field/Option Description
Sex * The patient's gender.
Ethnicity * (only for Human Cases) The patient's ethnicity.
Race (only for Human Cases) The race of the patient.

Date of Birth / Age Section:

To display this section on the Demographics tab page, click the header.

Field/Option Description
Date of Birth If available, the patient's birthdate.
Age (at Event Date) The approximate age of the patient at the time of disease onset when the patient's birthdate is not available.
Age Unit The measurement of the patient's age at the time of disease onset when the patient's birthdate is not available and they have the approximate age.
Country of Birth The country where the patient was born.

Patient Name Section:

This section is displayed only for human cases. To display this section on the Demographics tab page, click the header.

Field/Option Description
Name Type * The patient's name type.
Prefix The patient's name prefix.
Last * The patient's last name.
First * The patient's first name.
Middle The patient's middle name.
Suffix The patient's name extension.

Patient Alternate Name Section:

This section is displayed only for human cases. To display this section on the Demographics tab page, click the header.

Field/Option Description
Name Type The patient's alternate name type.
Prefix The patient's alternate name prefix.
Last The patient's alternate last name.
First The patient's alternate first name.
Middle The patient's alternate middle name.
Suffix The patient's alternate name extension.

Parent/Guardian Information Section:

To display this section on the Demographics tab page, click the header.

Field/Option Description
Last Name The last name of the patient's parent or guardian.
First Name The first name of the patient's parent or guardian.
Middle Name The middle name of the patient's parent or guardian..

Parent/Guardian Address

Street 1 The first line of the street address.
Street 2 The second line of the street address.
State The state name of the address.
NOTE: If the state is changed, the City, Zip and County/Borough are cleared. If the state is not the customer’s state, the County/Borough is disabled. If County/Borough is changed, the City and Zip fields are cleared.
City The city name of the address.
NOTE: If the state is changed, the City, Zip and County/Borough are cleared. If the state is not the customer’s state, the County/Borough is disabled. If County/Borough is changed, the City and Zip fields are cleared.
Zip The zip code of the address. This field is automatically populated by selecting the City field.
NOTE: If the state is changed, the City, Zip and County/Borough are cleared. If the state is not the customer’s state, the County/Borough is disabled. If County/Borough is changed, the City and Zip fields are cleared.
County/Borough The county/borough of the address.
NOTE: If the state is changed, the City, Zip and County/Borough are cleared. If the state is not the customer’s state, the County/Borough is disabled. If County/Borough is changed, the City and Zip fields are cleared.

Parent/Guardian Phone

Work Phone The work phone number of the patient's parent or guardian.
Ext The extension of the work phone number, if applicable.
Home Phone The home phone number of the patient's parent or guardian.
Ext The extension of the home phone number, if applicable.

Work/Occupation or School/Grade Section:

To display this section on the Demographics tab page, click the header.

Field/Option Description
Occupation Type (Select All That Apply) Select one or more occupations from the drop-down list.
Worksites/School Enter the additional details about the worksite or school based on the type of occupation selected in the Occupation Type field as it relates to possible sources of exposure or the spread of a disease.
Occupations/Grade Enter the additional details about the worksite occupation or school grade based on the type of occupation selected in the Occupation Type field as it relates to possible sources of exposure or the spread of a disease

Incarceration

Incarcerated Select Yes or No to indicate whether the patient was incarcerated.
Name of Correctional Facility If the Incarcerated field is set to Yes, enter the name of the correctional facility.
Prisoner Number If the Incarcerated field is set to Yes, enter the number of the prisoner.

Travel History Section:

To display this section on the Demographics tab page, click the header.

Field/Option Description
In the month prior to onset, did the patient travel outside of state? Select No, Unknown, or Yes to indicate if the patient has traveled outside of the state or not.
Destination (1st, 2nd, 3rd, 4th) If the In the month prior to onset, did the patient travel outside of state? field is set to Yes, enter the location name. You can enter up to four locations.
Country (1st, 2nd, 3rd, 4th) If the In the month prior to onset, did the patient travel outside of state? field is set to Yes, enter the name of the country. You can enter up to four countries.
Depart Date (1st, 2nd, 3rd, 4th) If the In the month prior to onset, did the patient travel outside of state? field is set to Yes, enter the start date of the travel or click in the field and select the date from the calendar pop-up. Today's date is the default date.
Return (1st, 2nd, 3rd, 4th) If the In the month prior to onset, did the patient travel outside of state? field is set to Yes, enter the return date of the travel or click in the field and select the date from the calendar pop-up. Today's date is the default date.
