The Lab Reports tab page contains summary information for each laboratory report (tests used to diagnose the disease) associated with the patient. The laboratory tests can be entered into Sentinel from two sources:
You can view both electronic and manual reports from the Lab Reports tab page, but you can only edit the manual lab report from the Lab Reports tab page.
For instructions on how to view or edit the Lab Reports tab page, see View/Edit Case Details.
The available fields, options, buttons, and columns on the Lab Reports tab page are as follows:

Create New Lab Report | Click this button to create a new manual lab report. See Create a New Manual Lab Report. |
Search | Enter the first few characters of the search name or date to narrow the list of results. The list automatically filters based on what is entered in the field. |
Date Received | The date the lab report was received. |
Lab Name | The Clinical Laboratory Improvement Amendments (CLIA) name for the laboratory where the specimen was tested. |
Lab Report Date | The date the lab report was completed. |
Source | The source of the lab report. Example: Electronic (ELR) or Manual. |
View | Click this button to view both electronic and manual lab report details. This button displays when you click the View button on the main Investigations tab page. See View an Electronic or Manual Lab Report. |
Edit | Click this button to edit an existing manual lab report. This button displays when you click the Edit button on the main Investigations tab page. See Edit a Manual Lab Report. |
Delete | Click this button to delete the lab reports. This button displays when you click the Edit button on the main Investigations tab page. See Delete a Manual Lab Report and Delete an Electronic Lab Report. |
To create a new manual lab report, first search for the case and/or locate it in the case list on the main Investigations tab page and then click the Edit button. The Case Reporting tab page opens. Click the Lab Reports tab, then click the Create New Lab Report button. The Lab Order Information page opens. Enter the lab report details and click the Save & Finish New Lab button. Click the Cancel button to return to the Lab Reports tab page.
NOTE: Facility users have access to create non-human cases and add lab reports. The lab reports are visible to the facility user until a non-facility user updates the lab report. |
The available fields, options, and buttons on the Lab Order Information page are as follows:

Laboratory Report Date | Click in the field to enter the date the laboratory report was completed or select it from the calendar pop-up. Today's date is the default date. | |

Name | Enter the name of the physician who ordered the lab report. | |
Phone | Enter the phone number of the physician who ordered the lab report. | |
Ordering Facility Name | Enter the facility name that ordered the lab report. Example: Hospital, Physician's office, clinic, etc. | |
Isolate sent to another lab(s) for additional testing | Select No, Unknown, or Yes from the drop-down list to indicate if the isolate was sent to another lab for testing or not. | |

Specimen ID | Enter the specimen ID. | |
Specimen Collection Date | Click in the field to enter the date the specimen was collected for testing or select it from the calendar pop-up. Today's date is the default date. | |
Specimen Site | Select the anatomical site from which the human specimen was collected from the drop-down list. | |
Specimen Source | Select the origin of the specimen/sample from the drop-down list. Example: Amniotic fluid, Aspirate, Biopsy, etc. | |
Organism Information |

Organism Name | Select the name of the organism from the drop-down list. | |
Organism Species | Select the organism species from the drop-down list.
This is the fundamental category of taxonomic classification, ranking below a genus or subgenus and consisting of related organisms capable of interbreeding. Example: For Herpes simplex, Herpes is the genus name and Herpes simplex describes a species (or category) of Herpes virus. |

Organism Serogroup | Select the organism serogroup from the drop-down list. It is used when an organism may have a serogroup designation. | |
Organism Serotype | Select the organism serotype from the drop-down list. It is used when an organism may have a serotype designation. | |
Organism Serotype Other | If Other is selected in the Organism Serotype field, enter the name of the serotype. | |
PFGE |

Pattern created by pulsed field gel electrophoresis (PFGE), used to identify genetically similar (and, therefore, potentially epidemiologically-linked) isolates. Enter up to three pattern areas in the following fields. | ||

A manual lab report is a laboratory report that you enter directly into the Lab Reports tab page for an existing case.
To edit a manual lab report, first search for the case and/or locate it in the list on the main Investigations tab page, then click the Edit button corresponding to the case. The Case Reporting tab page opens. Click the Lab Reports tab and locate the manual lab report you want to edit, then click the Edit button for that report. The Lab Order Information page opens. Make any necessary changes and click Submit Changes. Click the Cancel button to return to the Lab Reports tab page. See Create a New Manual Lab Report for available fields and their descriptions.
To add a new observation to an existing manual lab report, click the New Observation button in the Observations section. See Add a New Observation to a Manual Lab Report.
To delete a manual lab report, first search for the case and/or locate it in the case list on the main Investigations tab page, then click the Edit button corresponding to the case. The Case Reporting tab page opens. Click the Lab Reports tab, search for and/or locate the lab report you want to delete, and click the Delete button corresponding to the report. Click OK to confirm.
You can view both electronic and manual lab reports by clicking the View button on the main Investigations tab page for the case.
When the State Public Health Laboratory confirms the diagnosis of a disease, the laboratory sends this information to the application in form of an Electronic Laboratory Report (ELR). The ELR file is formatted using HL7 v. 2.5.1. ELRs are read-only; you cannot change them, but they can be deleted if they were appended or created incorrectly.

ELR files are processed several times per day via a background process that requires no intervention. The background process checks for the existence of an ELR file and processes the records contained in the file.
NOTE: If an ELR is received with an invalid state abbreviation, the state displays as Unknown. |
Each record that is received within an ELR file undergoes the same procedures as manually entered cases (geocoding and deduplication). Therefore, an ELR can create a new case within Sentinel or it can add information to an existing case.
NOTE: For electronic lab reports, the name of sending facility in the HL7 message is the laboratory name in the lab reports for an investigation. |
To view an electronic lab report, first search for a case and/or locate it in the case list on the main Investigations tab page, then click the View button corresponding to the case. The Case Reporting tab page opens. Click the Lab Reports tab, locate or search for the lab report in the list and click the View button corresponding to that report. The Lab Order Information page opens. To view the HL7 log entry, click the View HL7 Log Entry button. Click the Cancel button to return to the Lab Reports tab page. See Create a New Manual Lab Report for available fields and their descriptions.
To view the electronic HL7 log entry, click the View HL7 Log Entry button. The ELR Log pop-up window opens. Click the Close button to close the window.
The available fields on the ELR Log pop-up window are as follows:

Systematized Nomenclature of Medicine (SNOMED) codes, where available, are sent within the electronic laboratory reports. Within Sentinel, these codes are associated to a disease name. If a laboratory report does not have a SNOMED code or if the SNOMED code does not relate to a disease name, the laboratory report is assigned to an administrator for review in the pending work queue.
When a new case is initiated from an electronic laboratory report (ELR), the case must be assigned to a jurisdiction and to an investigator. In addition, electronic laboratory reports that are associated with an existing case may provide dissimilar address information. Case assignment and jurisdiction determination for an electronic laboratory report are handled as follows:
To view a manual lab report, first search for the case and/or locate it in the case list on the main Investigations tab page, and then click the View button corresponding to the case. The Case Reporting tab page opens. Click the Lab Reports tab, either search for or locate the lab report you want to view and click the View button corresponding to the report. The Lab Order Information page opens. Click the Cancel button to return to the Lab Reports tab page. See Create a New Manual Lab Report for available fields and their descriptions.
To delete an electronic lab report that was inadvertently appended or created, first search for the case and/or locate it in the case list by looking at the ELR column on the main Investigations tab page, then click the Edit button corresponding to the case. The ELR column displays either C or C A. The Case Reporting tab page opens. Click the Lab Reports tab, search for and/or locate the lab report you want to delete, and click the Delete button corresponding to the report. Click OK to confirm.
You can add a new observation to both new and existing manual lab reports.
To add a new observation to both new and existing manual lab reports, search for the case and/or locate it in the case list on the main Investigations tab page and click the Edit button. The Case Reporting tab page opens. Click the Lab Reports tab, then click one of the following buttons:

New Observation Section - If the facility has a lab observation mapping set for manual lab reports, the following fields are displayed in the New Observation section:

To edit an observation of a manual lab report, first search for the case and/or locate it in the case list on the main Investigations tab page, then click the Edit button corresponding to the case. The Case Reporting tab page opens. Click the Lab Reports tab and locate the manual lab report for which you want to edit the observation for, then click the Edit button. In the Observations section, either search for or locate the observation you want to edit, then click the Edit button. Make any necessary changes and click Submit Changes. To continue without making any changes, click Cancel and select OK to confirm. See Add a New Observation to a Manual Lab Report for available fields and their descriptions.
To delete an observation from a manual lab report, first search for the case and/or locate it in the case list on the main Investigations tab page, then click the Edit button corresponding to the case. The Case Reporting tab page opens. Click the Lab Reports tab and locate the manual lab report for which you want to delete an observation, then click the Edit button. In the Observations section, either search for or locate the observation you want to delete, then click the Delete button for the observation.
