CDC Form/Supplemental Forms Tab

The CDC Form/Supplemental Forms tab allows you to view any CDC supplemental forms associated with the disease or outbreak.

For instructions on how to view or edit the CDC Form/Supplemental Forms tab page, see View/Edit Case Details.

Supplemental forms can be associated with a disease or an outbreak. See Associate a Supplemental Form to a Disease and Associate a Supplemental Form to an Outbreak for more information.

The following tab names appear depending on the number and types of forms assigned to a case:

The available options on the page are as follows:

Select one of the following options to perform an action:
  • Case Definition - Opens the disease information in another browser tab or window and displays the Centers for Disease Control and Prevention (CDC) or other sources, such as Red Book Online
  • Vaccination Records - Opens the patient vaccination record from the registry application (if it exists) in another browser tab or window. This option is available only for human cases. See View Vaccination Records
  • Print [Form Name] - Select this option to print the CDC Form/Supplemental Form
  • Print Investigation - Select this option to print a summary of the case information. See Print a Case Investigation
  • Print Case Report - Select this option to print the case report. This option is available only for human cases
  • Print Physician Card - Select this option to print the case details entered by the physician card user. This option is available when the case has been entered by a physician card user
  • Debug NETSS CDC Export - Opens a pop-up window that lists all the fields that are being sent to the CDC (only CDC form fields are listed here). This option is available when the disease of the opened investigation is exported using NETSS (configured in CDC Export Mapping). It is displayed in the drop-down arrow button only when the Embedded Form Debug permission is granted in the Administrative Debug Permissions section and the NETSS Debug Mode is enabled from the Page Configuration button.   • Debug HL7 CDC Export - Opens a new browser tab or window to display the fields that are being sent to the CDC. This option is available when the disease of the opened investigation is exported using any other method than NETSS and is displayed when the HL7 CDC Debug Export permission is granted in the Administrative Debug Permissions section.   • Debug HL7 CDC Export (Basic) - This option is visible when the HL7 CDC Export Debug Mode is enabled from the Page Configuration button. Opens a new pop-up window to display basic information about the fields that are being sent to the CDC
  • Debug CSV Export Columns - Opens the CSV Debug pop-up window that lists all the columns exported in the CSV export of the CDC form or supplemental form
  • Debug Database for Patient - This option is visible when the Debug Mode permission is granted in the Administrative Debug Permissions section. It displays the database information about the patient. Contact STC to use this
  • Debug Database for Case - This option is visible when the Debug Mode permission is granted in the Administrative Debug Permissions section. It displays the database information about the case. Contact STC to use this

CDC Form or [Supplemental Form Name] Tab

If only one form is associated with a case, the tab name may be CDC Form, Supplemental Form, or [Form name] (if a custom form).

CDC Form

Every CDC form contains the following information: Investigator Name, Agency, Phone Number, Date of Investigation, and Comments or Additional Information.

To edit the data on the CDC Form, first search for the case and/or locate it in the case list on the main Investigations tab page, then click the Edit button for the case. The Case Reporting tab page opens. Click the CDC Form tab. Make any necessary changes and click Submit Changes.

The CDC data tab page shows all the active diseases and their corresponding forms for the State Health Department, respective to each state.

NOTE: Since every disease form differs, not every form is documented in this section.

The CDC tab page displays the CDC disease fields for all cases that have existing forms. The CDC Data Communicable Disease Form for the specific disease case appears, if one is available (fields differ per disease/state). The list below describes some example forms:

For West Nile Virus:

The Published Indicator is a required field and the options are Yes or No. If the Died Due to this Condition field is set to Yes, the Date of Death is entered on the Case Reporting tab page. The Culture Result field has the following options in the drop-down list: Positive, Negative, Not Done, and Unknown.

For Q Fever:

The travel outside of the U.S. information displays on the Demographics tab page. Also, the Investigator Name, Investigation Date, Investigator Phone Number, Investigator Agency, and Comments appear in the Additional Information section.

For CJD (Creutzfeldt-Jakob Disease):

The month and year of initial symptoms is the Onset Date on the Case Reporting tab page. The Patient's Code Number is the Medical Record Number on the Case Reporting tab page. Date Form Filled Out is the Date of Investigation. Comments are added in the additional information section of the form.

Supplemental Form/[Form Name]

To edit the data in the Supplemental Form/[Form Name], first search for the case and/or locate it in the case list on the main Investigations tab page, then click the Edit button for the case. The Case Reporting tab page opens. Click the Supplemental Forms or [Form Name] tab. Make any necessary changes and click Submit Changes.

If more than one form is associated with a case, a tab with the name Supplemental Forms appears. When you click on the Supplemental Forms tab, a new row of tabs appears underneath, one for each of the forms associated with the case. The new row includes:

The form that opens first is the form that was the first to be associated with the disease. If no supplemental form is associated with the disease, the first form to be associated with the outbreak opens. The second row of tabs displays either the CDC Form or the form defined in Form Management. Note that if you hover the mouse pointer over on any tab on the second row, a tool-tip appears with the full name of the form.

Search for Cases Using Forms

You can search for cases using either the CDC Form or a Supplemental Form.

Search for Cases Using a CDC Form

To conduct a search using the CDC form associated with a disease without entering any search criteria on the search page, click the Search button on the main Investigations tab page, select the CDC form you want to search by from the Investigation Search drop-down list, and then click the Search button. The main Investigations tab page opens with the search results.

Search for Cases Using a Supplemental Form

The supplemental forms are assigned to a disease and to an outbreak.

Use a Supplemental Form Associated with a Disease

To conduct a search using the supplemental form associated with a disease without entering any search criteria on the search page, click the Search button on the main Investigations tab page, select the supplemental form you want to search by from the Investigation Search drop-down list, and then click the Search button. The main Investigations tab page opens with the search results.

NOTE: The Disease Name field shows the diseases that are associated with the supplemental form.

Use a Supplemental Form Associated with an Outbreak

To conduct a search using the supplemental form associated with an outbreak without entering any search criteria on the search page, click the Search button on the main Investigations tab page, select the supplemental form you want to search by from the Investigation Search drop-down list, and then click the Search button. The main Investigations tab page opens with the search results.

NOTE:  The Outbreak Name field shows the outbreaks that are associated with the supplemental form.
