Immunization History Registry Report (Mississippi)

This report is produced as a CSV file and contains the following fields:

  • Report Date
  • SIIS ID
  • Date Administered
  • Vaccine Name
  • Vaccine MIIX Code
  • Vaccine CVX Code
  • Vaccine Family
  • Vaccine Family Code
  • Age at Dose
  • Age Indicator
  • Invalid Dose Indicator
  • Historical Indicator
  • VFC Code
  • VFC PIN (Yes/No)
  • Administered by Facility Name
  • Administered by Facility District/Region
  • Date Entered in MIIX
  • Username

To run this report, click the Reports > State Reports > Other > Immunization History Registry Report link. The Immunization History Registry Report Menu page opens.  Enter the birth date range From and Through dates and click Export. If any data is available for the specified date range, a CSV file automatically downloads and can be opened by a spreadsheet program such as Excel.
