Vaccine Preferences by Dose to
Override ACIP Schedule Recommendations
The purpose of Vaccine Preferences by Dose to Override ACIP Schedule
Recommendations is to allow the administrator to change the core forecast
for a vaccine family.
Example of Use
In August 2009, the ACIP recommendation for polio was modified. This
modification affected at what age dose 4 is the valid and final dose.
However, before this modification came into effect, ACIP guidelines allowed
dose 4 to be administered as the final dose earlier than 4 years of age.
Some states may have state school laws, and so on, that require the final
dose of polio to be administered at >= 4 years of age regardless of
past ACIP recommendations. The vaccine preferences by dose settings enable
that type of override for state-specific criteria.
Let's use this as an example: DOB=01/01/2005 and an administered dose
4 of IPV on 09/01/2008. This is counted as a valid and final dose 4 under
ACIP guidelines because it was administered prior to the published change.
To enforce that dose 4 be administered at >= 4 years of age, the recommended
and minimum age settings may be configured with Vaccine Preferences by
Dose. It is only necessary to complete the fields relevant to the particular
scenario. In this case, recommended age and minimum age would be configured
for 48 months of age.
To access the Vaccine Preferences by Dose to Override ACIP Schedule
Recommendations section, click on the Forecasting
Settings link on the main page, then scroll down the page. Enter
the parameters and click the Save
button. To remove the vaccine preference, click the Delete
icon displayed in the row for the vaccine.

The available fields and options in the Vaccine Preferences by Dose
to Override ACIP Schedule Recommendations section are as follows:

Vaccine Family |
Select the vaccine family name to be modified from the
drop-down list. |
Dose Number |
Select the dose number of the vaccine to be modified from
the drop-down list. |
Recommended Age |
Enter the recommended age in the first field and then select
either Days or Months from the drop-down list.
Note: The
recommended age is the age at which the vaccine is
recommended. This is used as a recommendation only
and is not the minimum date against which a vaccine
is validated as meeting a minimum age or interval. |

Minimum Age |
Enter the minimum age in the first field and then select
either Days or Months from the drop-down list.
Note: The
minimum age is the earliest age set by the state for
which this vaccine dose is considered valid. |

Minimum Interval from
Previous Dose |
Enter the minimum interval from the previous dose in the
first field and then select either Days or Months from the
drop-down list.
Note: The
minimum interval allows the state to enforce an interval
from the previous dose which overrides ACIP guidelines.
For example, Varicella vaccine administered to children
>= 13 years of age may have a week's interval between
doses. To enforce a 12-week interval instead, set
this to 84 days. |

Recommended Interval from
Previous Dose |
Enter the recommended interval from the previous dose in
the first field and then select either Days or Months from
the drop-down list.
Note: The
recommended interval allows the state to display a
recommended date based on an interval from the previous
dose which overrides ACIP guidelines. For example,
Varicella vaccine administered to children >=13
years of age may have a 4-week interval between doses.
To display the recommended date with a 12-week interval
instead, set this to 84 days. |

Past Due Age |
Enter the past due age in the first field and then select
either Days or Months from the drop-down list.
Note: The
past due age allows the state to calculate a past
due age based on its own criteria. For example, DTaP
#5 is recommended at age 4-6 years and would display
as overdue at 7 years of age. This setting can be
used to calculate the past due age at 5 years of age
instead, if that is the preferred setting. |

Continue Forecasting after
ACIP Completion |
Select this option to continue forecasting for this vaccine
even after the series is met. This can be used for vaccine
families, such as for PPSV, which are generally a one-dose
series. It is not used for a series with scheduled booster
doses, such as Td.
This setting requires that the vaccine family,
dose number, recommended interval from previous dose, and
the continue forecasting after ACIP completion settings all
be set for each subsequent dose that should be forecast. |
