Vaccination Warnings and Invalid Reason Descriptions

If an immunization does not meet criteria during the immunization history evaluation process, vaccination warnings and invalid reasons identify why.

The warnings indicates that the immunization is acceptable in the series and does not need to be repeated, but was perhaps given as off-label (not meeting the vaccine licensure recommendation), or at an interval earlier than the recommended minimum interval.

Immunization warnings are based on guidance from the CDC National Immunization Program and may not be documented in the ACIP Guidelines.

An immunization flagged with an invalid vaccination reason did not meet the minimum criteria for satisfying the dose in the series schedule and must be repeated.

The following table contains vaccination warning and invalid reason descriptions:

INVALID Live vaccines not administered on same date must be separated by 28 days.
INVALID Minimum interval from previous dose not met.
INVALID Vaccine not licensed in U.S.
INVALID HPV Bivalent vaccine is not appropriate for male vaccination or cannot be validated due to missing gender.
INVALID Tdap administered at < 7 years of age and as dose 1, 2, or 3 should be repeated.
INVALID Minimum interval from Dose 1 not met.
INVALID Minimum age for this dose was not met.
INVALID Measles or Varicella containing vaccine administered < 14 days prior to antibody containing product.
INVALID Measles or Varicella containing vaccine administered less than recommended interval following antibody containing product.
INVALID Antigen not found for this schedule.
INVALID Vaccine marked as Compromised.
INVALID Antigen recommended for booster dose only.
INVALID Antigen not recommended for primary series.
INVALID Patient age outside of recommended schedule.
WARNING Seasonal Influenza Nasal and Novel Influ-H1N1-09 Nasal should not be administered at same visit but CDC does not recommend repeating the doses.
WARNING Influenza 9+ years administered to patient under 9 years of age.
WARNING Tdap administered at < 7 years of age and dose number > 3 but CDC does not recommend repeating the dose.
WARNING PCV13 is recommended for children ages 14-59 mos. previously complete for PCV7.
WARNING Rotavirus Dose 1 administered at age greater than recommended age. CDC recommends completing series.
WARNING The minimum recommended interval between Td and Tdap is 5 years. However, shorter intervals may be counted as valid.
WARNING DTaP-Hib-IPV Dose 4 administered at < 4 years of age does not satisfy the IPV dose 4 minimum age. Another IPV should be administered at >= 4 years of age.
WARNING Patient may be on Twinrix alternative schedule.
WARNING Minimum interval from previous dose not met but CDC does not recommend repeating this dose.
WARNING The minimum recommended interval between DTaP-3 and DTaP-4 is 6 months. However, DTaP-4 need not be repeated if administered at least 4 months after DTaP-3.
WARNING Antigen administered off-label but CDC does not recommend repeating dose.
WARNING Influenza, 6-35 months administered at < 6months of age or > 35 months of age.
WARNING Influenza, 36+ months administered at < 36 months of age.
WARNING Tdap administered at > 64 years of age.
WARNING Tdap administered at < 10 years of age.
WARNING DTaP administered at >= 7 years of age.
WARNING PPV23 administered at < 24 months of age.
WARNING Td administered at < 7 years of age.
WARNING DTaP/IPV is not approved for doses 1 through 3 of IPV but CDC does not recommend repeating the dose.
WARNING DTaP/IPV is not approved for doses 1 through 4 of DTaP but CDC does not recommend repeating the dose.
WARNING DTaP/HepB/IPV is not approved for doses 4 or 5 of DTaP or IPV but CDC does not recommend repeating the dose.
WARNING Herpes Zoster administered to person < 50 years of age but CDC does not recommend repeating the dose.
WARNING Pediatric/Adolescent vaccine recorded as administered to adult >=19 years of age.
WARNING MMR/Varicella administered at >=13 years of age.
WARNING TIV (Flu) High-Dose administered at < 65 years of age.
WARNING DTaP/Hib/IPV should not be administered as dose 5 DTaP or to children >=5 years of age but CDC does not recommend repeating the dose.
WARNING Influenza, 18+ years administered at < 18 years of age.
WARNING Influenza, 48+ months administered at < 48 months of age.
WARNING Multiple doses administered for single dose series. Revaccination for meningococcal recommended for high risk only.
WARNING Multiple doses administered for single dose series.
WARNING Multiple doses administered for single dose series. One dose of Tdap is recommended.
WARNING Antigen not recommended for primary Hib series and CDC recommends disregarding in forecast.
