Patient List Report (Custom)

This Patient List Report allows users to run the Patient List report based on custom filters/parameters, with an age range between 0 and 19 years. The report can be generated, printed, and exported, and the report template can be saved.

To access the Patient List Report, click the Custom menu link. The Create Custom Patient List Report page opens. Select the appropriate filters and options, then click the Preview icon (Run Report icon). The Filtering By section at the top displays the current filtering criteria. To clear the report filters and options (parameters) and start over, click the Reset Filters icon (Clear Filters icon). The current filtering criteria displays at the top of the page.

Note that some filter sections can be expanded or collapsed (hidden) by clicking the down or up arrow above the filter name, respectively. Also, the actual filters are listed at the top of the page, in the Filtering By section.

The following filters and options are available for this report:

Filter/Option Description

Provider

Enter the provider name or VFC PIN (if not pre-populated). If a pre-populated organization/facility name displays an X, click it to remove the name and enter another.

Provider Relationship

This is a configurable parameter at the State level that is not visible to all states. It is unique to independent states with a 1:M patient active/inactive status (PAIS) structure. Select either All Active Patients or Active Patients Owned by the Facility.

As of Date

From the drop-down list, select the number of days in the past to run the report for (0-14, where 0 is today and 14 is from two weeks ago until today).

Age

Select an age option:

  • AFIX Childhood: from 24-35 months

  • AFIX Adolescent: from 13-17 years

  • Select Custom Range - select this option, then select the custom age range (select the From and To numbers, then select whether the numbers represent months or years). Once the custom age range is selected, the Select Custom Range displays the time period in parenthesis next to the main option. Example: Select Custom Range (Current: from 11 to 19 Years)

Vaccination

To add a vaccine, antigen, or series, enter or select the  name. To add an additional vaccine, click the + (plus) icon. To remove a vaccine, click the - (minus) icon.

VFC Eligibility Status

The default option is to include all VFC eligibility statuses. To narrow by eligibility status, select a status from the drop-down list. To add an additional status, click the + (plus) icon (which appears once a status other than All Status is selected). To remove a status, click the - (minus) icon.

Patient Demographics: Gender

The default is to include all genders. To narrow by gender, select it from the drop-down list. To select an additional gender, click the + (plus) icon and select it. To remove a gender, click the - (minus) icon.

Patient Demographics: Race

The default is to include all races. To narrow by race, select a race from the drop-down list. To select an additional race, click the + (plus) icon and select it. To remove a race, click the - (minus) icon.

Patient Demographics: Ethnicity

The default is to include all ethnicities. To narrow by ethnicity, select an ethnicity from the drop-down list. To select an additional ethnicity, click the + (plus) icon and select it. To remove an ethnicity, click the - (minus) icon.

After clicking the Preview icon (Run Report icon), the (possibly truncated) report displays in a pop-up window.

Save a Report Template

To save a report template to use again in the future, click the Save icon (Save Report icon). A pop-up window opens. Enter a descriptive name for the report and click Save. If there are any errors related to the selected report criteria, a message pops up and the area on the page with errors is highlighted. After successfully saving the report template, the name appears in the menu in the Custom reports section, below any previously created groups. To re-run the report again in the future, click the report template name in the menu (or within the group) and then click the Preview icon (Run Report icon). Run a Saved Report

To run a saved report template again in the future, click the report template name menu link (note that it might be listed under a report group name category on the menu).
