This report is also referred to as the Case Listing Report or the Line List of Disease Cases for a Given Timeframe and Geographic Location. It displays a patient-level list of all cases that fall within the specified time period, geographic area, and disease information as defined by the report parameters. The information included in the report is determined by the selected display column options.
NOTE: The Disease Name #2 field is enabled only when one - and only one - disease name is selected and that disease has secondary diseases. |
This report produces CSV output that varies from the other outputs for the same report due to the limitations in the number of columns supported by many spreadsheet applications. Multiple lines are displayed for cases that have multiple lab reports - that is, one line per lab report.
To run this report, select the Reports > Line List menu link, which opens the Line List of Disease Case for a Given Timeframe and Geographic Location page. Enter the report parameter options and click View CSV Report. The CSV report file is automatically downloaded. To reset all of the options to their default values and start over, click Reset.
The available fields and options for this report are as follows:
Field/Option | Description |
Time Period | Click in the From and To Date fields and select the dates from the calendar that opens. These fields are required. |
Geographic Area | Select a geographic area: State Wide (default), Nation Wide, Out of State, Zip (and enter a zip code), County (select a county from the list box), or District (select from the list box). |
Report Selection Criteria | To limit the report to a specific Case Classification or Investigation Status, select it and then select a classification or status from the related list box. Also select a Disease Name from the list box. |
Display Columns | Select all of the columns that should be displayed in the report. |
Lab-Related Display Columns | If any lab-related information should be included in the report, select the columns to include. |
