Provider

The Provider section of the menu allows users easy access to provider-related information during the onboarding process.

For State users, the Provider category includes links to provider-related pages:

• Provider List
• Provider Profile
• Interface Form

For Organization/Facility users, the Provider menu link takes the user to a page that includes tab sections for:

• Profile - Provider and contact information including address, phone and fax numbers, and email address.
• Interface Form - Once a State user assigns an interface form to the provider, it becomes accessible in the Interface Form tab section of this Provider page. Organization/Facility users can click on a tab section in the header (for example: Provider Info, EHR Info, MU & Status, Bi-Direction), enter responses for each question, and submit the form to the State. Once the form has been submitted, the information is stored here for future access and review.
