Disease by Geographic Area Report

The Disease by Area report displays count information for all diseases that fall within the selected time period and geographic area defined by the report parameters. The count or rate information can be displayed by geographic boundaries and areas. The report can also be run based on facility type or facility ID.

NOTE: The Disease Name #2 field is enabled only when one - and only one - disease name is selected and that disease has secondary diseases.

To run this report, select the Reports > Disease by Area menu link, which opens the Table of Diseases by Geographic Area page. Enter the report parameter options and click View HTML Report, View PDF Report, or View CSV Report, depending on the file format in which you want to review the report. To reset all of the options to their default values and start over, click Reset.

Field/Option	Description
Table Type	The Counts table type is automatically selected.
Time Period	Select Month (and select the month from the drop-down list and enter the year; the current year is the default), MMWR week (and enter the week number, 1-52, and year; the current year is the default), or From/To Date range (and enter the From and To dates). Month is the default.
Report Selection Criteria	To limit the report to a specific Case Classification or Investigation Status, select it and then select a classification or status from the related list box. Also select a disease name or ALL from the Disease Name list box.
Geographic Breakdown	Select a geographic breakdown area: State Wide, County, Local Health Department, or Zip.
Geographic Area	Select a geographic area: State Wide, Nation Wide, Out of State, Zip (and enter a zip code), County (and select a county or ALL from the list box), District (select a district from the list box), Facility Type (select from the list box), or Facility ID (select from the list box).
