This report is state-configurable and may not appear for all users.
The Summary Disease report displays count information for all cases that fall within the specified year and preceding four years. The count is displayed by borough and can be broken down by time period.
To run this report, select the Reports > Summary Disease Report menu link. (If the link does not appear on the Reports menu, it is not available.) Enter the report parameter options and click View HTML Report, View PDF Report, or View CSV Report, depending on the file format in which you want to review the report. To reset all of the options to their default values and start over, click Reset.
The available fields and options for this report are as follows:

| Borough | Select the borough from the list box. |
| Time Period | Select an option: Begin Period Week Ending (enter begin and end period dates) or Begin MMWR (enter begin and end week numbers, 1-52). |
