The Physician Card Staging tab page displays the list of staging entries stored in the application. A staging entry is created by someone who is not a Sentinel user, but has submitted a physician card form using the No-Login Physician Card link. The No-Login Physician Card link is used for online reporting of communicable disease without the need to login into Sentinel. Note that the No-Login Physician Card link can be activated only after a new user with the EXTERNALONLINEREPORT user ID is created in the application. See Outbreak Event Online Reporting for more information on how to create the EXTERNALONLINEREPORT user.
NOTE: State administrators determine whether or not the No-Login Physician Card link is shared on a webpage or via email. |
Users with the Physician Card Staging Area permission (granted in Administrative PWQ Permissions) can access the Physician Card Staging tab on the Home page and can view, delete, or create an investigation for the physician card staging entries stored in the application.
To access the Physician Card Staging tab, click the Home > Physician Card Staging tab. The displayed fields can be selected by clicking the Show/Hide button and then selecting the fields to display. The list of staging entries can be sorted by clicking on a column header. Under the list of current physician card staging entries, the number of entries displayed on the page is listed (for example, Showing 1 to 3 of 3 entries) and on the right side of that line are page number buttons. Click the page numbers and Next and Last buttons as needed to display any additional entries.

If you are permitted by the state to send the link to a person to request
them to fill out the physician card form, click the Home
> Physician Card Staging
tab, copy the No-Login Physician Card link from the Info icon () and paste it
in the email body, and then send it to the person via email. The recipient
can access the link from the email, fill out the form, and submit it to
the application without logging into Sentinel.
If a person is using the No-Login Physician Card link to submit a physician card, they are required to enter Captcha text in the Answer field for verification before clicking the Validate and Submit button.
Once a person submits a physician card using the No-Login Physician Card link, the physician card staging entry is created in Sentinel and listed on the Physician Card Staging tab page. You can review the submitted information by clicking the View icon on the Physician Card Staging tab page. See Create a New Physician Card for the available fields and their descriptions. You can also delete the staging entry (Delete a Physician Card Staging Entry) or create an investigation for the staging entry (Create an Investigation for the Physician Card Staging Entry).
There are three methods you can use to create an investigation for the physician card staging entry on the Physician Card Staging tab page. Note that once the investigation is created for the staging entry, the staging entry is deleted from the physician card staging entry list and added to the case list.
Note that once the investigation is created for a selected staging entry, it is displayed in the case list and you can view the summary of the new investigation on the Audit Tab page. After it is created, you can also search for the investigation using the Physician Card Staging ID on the Physician Card search.
