School Nurse Module Patient Detail

To view or edit patient demographics, a patient search must be performed first. To view the patient's demographic, click on the patient's row in the search results. The Patient Detail page opens.

School Nurse users should always see the most current demographic information for a patient, ignoring privacy rules which is a state configurable administration property. If the state does not allow School Nurses to see private information, then only private fields entered by the nurse users will be shown.

The School and Grade fields, and the Include on Reports option, can be updated on this page. After updating these fields, click Update to save the changes.

To edit patient demographic information, click Edit. The School Nurse Patient Demographics Edit page opens. Make changes as necessary and click Save. See Add/Edit Patient Records for more information about editing patient records and for the available fields and descriptions.

Field Description

Grade

Enter the patient's grade in school.

School Imms Status

Automatically generated based on the patient's school entry date and the vaccines they have received.
  • Compliant - The patient record contains all required vaccines.
  • Out of Compliance - The patient is missing one or more vaccines, does not have an exemption on file, and it has been more than 30 days since the school entry date.
  • Conditional - The patient is missing one or more vaccines, does not have an exemption on file, and it has been less than 30 days since the school entry date. This status changes to Out of Compliance 30 days after the school entry date.

Include on Reports

Select this option to include the patient in school reports. The default is enabled (selected), indicating the patient will be included on school reports. The default is disabled (deselected), indicating the patient will not be included on school reports.

School Exemptions by Disease

This section is split into four different types of exemptions that can be added to the school record. For specific definitions of exemption types, refer to the Washington State Department of Health's Certificate of Exemption form. All required fields are in red.

  • Medical Exemption - There are two types of medical exemption: Permanent (select the option) or Temporary Until (enter the date). Also select a vaccine from the drop-down list and enter a parent signature date (required).

  • Personal Exemption - Select a vaccine from the drop-down list and enter the parent signature date.

  • Religious Exemption - Select a vaccine from the drop-down list and enter the parent signature date.

  • Religious Membership Exemption - Select a vaccine from the drop-down list and enter the parent signature date.

NOTE: When a School Nurse adds an exemption, it is not passed on to the patient's vaccination record in IWeb.
