Childhood Patient List Report

To view the patient list for the Childhood Cohort, first select the facility in the search bar (if necessary) and then click the Childhood > Patient List menu link. The Childhood Patient List Report page opens with a preview of the list of patients aged 24 through 35 months (with related information) for that facility, in alphabetical order.

Note icon  The "Full List" filter for the Childhood Patient List report displays the list of patients as a truncated list on the screen. To view the entire full list of patients, export the data to a file using the Export icon (Export icon). The exported file also includes additional fields, such as the patient's phone number.

The Filtering By section at the top displays how the report was configured. The Childhood Patient List Report criteria is pre-set as listed below; to use different report criteria, run a custom Patient List Report.

Filter Type Criteria

Provider

The organization/facility displayed in the top search bar.

Ages Assessed

24 to 35 months of age.

As of Date

The current date.

Selected Cohort/Series/Antigens

  • 4 DTaP

  • 3 Polio

  • 1 MMR

  • UTD1 Hib

  • UTD Hep B

  • 1 VAR

  • UTD PCV

  • UTD RV Flu

  • 2 Hep A

1 UTD refers to the term up to date for measurements where a variable number of doses can be applied based on age, date of first dose, and/or vaccine product licensure.

Assessment Date

The date of assessment.

From the drop-down list, select which data to display:

The information displayed for each patient depends on the filter type (full list, missed opportunities, or invalid doses). Note that for missed opportunities and invalid doses, a patient may be listed more than once if he or she has missed opportunities or invalid doses for more than one antigen.

Full List

Patient Name

The name of the patient, displayed with the last name first and then the first name.

ID

The patient's IIS ID number.

DOB

The patient's date of birth.

Gender

The patient's gender.

Date of Last Vaccination

The patient's last vaccination date. Note that this date reflects the last vaccination for the patient overall, not necessarily at the selected facility. Also, a patient may have a different last vaccination date listed for flu compared to other antigens.

Missed Opportunities

Patient Name

The name of the patient, displayed with the last name first and then the first name.

ID

The patient's IIS ID number.

DOB

The patient's date of birth.

Vaccine

The name of the missed vaccine.

Dose Missed

The number of the missed vaccine dose. For example, Dose Missed: 1 indicates the patient missed the first dose in the series.

Last Immunization Visit

The date of the patient's last immunization visit. Note that this date reflects the last immunization visit for the patient overall, not necessarily at the selected facility. Also, a patient may have a different last vaccination date listed for flu compared to other antigens.

Invalid Doses

Patient Name

The name of the patient, displayed with the last name first and then the first name.

ID

The patient's IIS ID number.

DOB

The patient's date of birth.

Date Given

The date the invalid dose of the vaccine was given.

Reason Not Valid

The reason the dose given was not valid.

Antigen

The name of the invalid antigen given.

Additional patient details are included in the exported report (which also includes header information about the report):

The patient's phone number.

The name of the invalid antigen given.

Invalid Reason

The reason the dose given was not valid.

The date the invalid dose was given.

The patient's phone number.

The patient's cell phone number.

To print the report, click the Print icon (Print icon) and follow the prompts to select a printer and options. The Childhood Patient List Report prints as it displays on the screen.

To export the data to a file, click the Export icon (Export icon) and then click the type of file format to export as: CSV or XLS. To cancel the export process, click either the Cancel Export button or the X in the upper right-hand corner of the Exporting Report pop-up window.
